Maplewood Health Department Offers Free Medication Reconciliation April 17

On April 17th at 2:30 pm, Maplewood Health Department and St. Barnabas Hospital will offer a free public medication reconciliation. The reconciliation takes an accurate account of all medications a person takes to ensure correct prescription. The 10-minute appointments will be held at Winchester Gardens Auditorium at 333 Elmwood Avenue, Maplewood.

The reconciliation will be facilitated by Jessica Bente of Saint Barnabas Medical Center in Livingston, NJ. Jessica Bente, Transitions of Care pharmacist at Saint Barnabas Medical Center, is a licensed pharmacist and registered immunizer. She received her PharmD from St. John’s University in May 2014, after which she pursued a postgraduate year one (PGY-1) pharmacy practice practice residency at Saint Barnabas Medical Center. Being a part of the Transitions of Care Team, Jessica primarily follows patients with a diagnosis of COPD or pneumonia, who return home upon discharge. Her primary responsibilities include medication reconciliation, medication counseling, follow-up phone calls, and home visits to patients once discharged. Her practice areas of interest are COPD, dyslipidemia, and diabetes management.
